The Life Long Care Program

The Life Long Care Program was established at the Cochrane site in June 1995. The objective of the program is to assist a target group consisting of off-reserve Aboriginal Elderly/Seniors, mentally and physically challenged, chronically and terminally ill. In September 1999 a secondary worker was implemented. The LLC Program was then distinguished into two segments; the Elderly/Seniors and the non-Elderly/non-Seniors. The purpose of this design is to give a better quality of service to the target group which administers interpretation to the Elderly/Seniors.

The LLC program components consist of...

TRANSPORTATION

Provided to clients for their various medical appointments to their family physician, hospital, dentists, physiotherapy, optometrist, and so on.

ABORIGINAL SUPPORT

Advocating on the clients behalf to various external agencies such as family physicians, landlords, pharmacists, Medical Services, Ontario Work, Ontario Disability Support Program, HRDC, CPP & Old Age, referrals to CCAC, etc.

MEALS PROGRAM

Consist of soup kitchen every Wednesday, Congregate dining which includes a meal / activity / transportation

FRIENDLY VISITS

Delivered in the home, Villa Minto, hospital and the Friendship Centre

NOTE; friendly visits are designed to be delivered by volunteers
